Case Manager
Minimum Qualifications
Bachelor’s degree in health, human or education services and a minimum of one year of qualifying experience including case management or casework with persons who have HIV infection, a history of mental illness, homelessness or chemical dependence, OR an Associate's degree in health or human services or certification as an R.N. or L.P.N. and two years of qualifying experience. Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions. Ability to speak Spanish and/or ASL preferred. Must have a reliable vehicle and meet the agency driving policy requirements for this position, including a valid NYS driver’s license and proof of required automobile insurance. Must be able to lift, load and unload up to 25lbs.
Responsibilities include but are not limited to:
- Ensure intakes, assessments, reassessments, service plans, and progress notes are completed and submitted to supervisor within designated time frames.
- Develop and implement comprehensive individualized service plans for clients and collaterals.
- Ensure service plans reflect ongoing needs and progress of clients and collaterals.
- Ensure service plans reflect specific outcomes for goals and tasks.
- Document each client interaction comprehensively and concisely.
- Advocate for client access of entitlement programs, treatment facilities, and other needed community services.
- Coordinate services with community agencies and medical facilities to provide unduplicated professional case management.
- Schedule and/or participate in case conferences when changes in mental/health status occur and at time of reassessment.
- Promote compliance with medical appointments and treatment adherence.
- Promote client access into internal agency services.
- Coordinate and transport clients to services within AIDS Rochester’s eight county service area as needed.
- Compile statistics and other information upon request.
- Demonstrate developing knowledge of community resources.
- Demonstrate developing knowledge of HIV/AIDS and related issues.
- Adhere to all government and funder regulations.
- Provide and document the required billable hours of client services.
- Other duties as assigned by supervisor.
SALARY: $28,000
